- For cases with State Agencies or Departments, download the Constituent Casework Authorization Intake Form in English or in Spanish.
- For Employment Development Department (EDD)-specific cases, download the EDD Casework Authorization Form.
Once you’ve completed your casework form, submit it to our office by emailing assemblymember.caloza@assembly.ca.gov with the subject line “CASEWORK: First Name Last Name DD/MM/YY.”
